FINANCIAL & INSURANCE INFORMATION FROM YOUR PORT COQUITLAM DENTAL PRACTICE

We accept VISA, MasterCard, American Express, and cash. Should you require payment plan information, please inquire at the front desk.

At Oxford Family Dental, we accept most dental insurance programs from all major insurance providers. Please note that, upon arrival, our administrative department will prepare all necessary paperwork and forms to correctly administer your dental benefits. If you prefer to fill these documents out prior to your appointment, click here to download two forms, patient medical history and insurance coverage.

We kindly remind you that your individual policy belongs to you and that policies vary greatly between services and amounts covered based on provider. You are responsible for any fees not covered by your insurance plan. If you would like a pre-treatment estimate submitted to your insurance company, let your Port Coquitlam dentist know, and we can find out what your coverage entails. All other inquiries should be directed to your benefits provider.
